This Subject Number replaces Subject Number 046-756, which used an incorrect Authorization Number. The correct Authorization Number is 197094-6W. Please disregard Subject Number 046-756.

Please be advised that Dr. Christopher L. Wright has resigned his authorization to treat injured workers in the New York State workers’ compensation system effective March 3, 2015. Dr. Wright’s last known practice address is 2 Coulter Road, Clifton Springs, NY 14432.

Dr. Wright is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Wright for services rendered prior to March 3, 2015, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Wright for services rendered prior to March 3, 2015, should not be denied due to his resignation.
