Subject Numbers Regarding Health Provider Authorizations

Please be advised that, effective October 23, 2008, Steven F. Gass, D.P.M.'s authorization (Number P02943–0W) to treat workers' compensation claimants was temporarily suspended pursuant to Workers' Compensation Law §13–k(12). Dr. Gass' last known address is 100 West Broadway, Apt. 3V, Long Beach, New York 11561. The suspension is effective until October 22, 2009.

While the suspension is in effect, Dr. Gass is prohibited from rendering podiatric treatment and care to individuals who have suffered work–related injuries. Medical reports submitted for treatment rendered prior to October 23, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Gass regarding treatment he rendered prior to October 23, 2008, should not be denied due to this suspension.
