Please be advised that effective February 2, 2006, James Lyle Latimer, M.D., voluntarily resigned his authorization (number 150723-5B) to treat and to conduct independent medical examinations of workers' compensation claimants. Dr. Latimer's last known address is 90 Leroy Street, Potsdam, New York 13676.

Dr. Latimer is prohibited from rendering medical or other treatment and care to, and conducting independent medical examinations of, individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Latimer for treatment or examinations rendered prior to February 2, 2006, are valid, but are invalid for any treatment or examinations rendered after that date. Requests to cross-examine Dr. Latimer regarding treatment or examinations he rendered prior to the resignation's effective date should not be denied due to this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through February 1, 2006, may be provided.
